Pre-Test - Students will complete a short pre-test using a Google Form.
Act 1 Guide Sheet - This guide provides the structure for Act 1. This guide measures students ability to create an equation, graph the equation and interpret the results of the graph. This meets standards CCSS.MATH.CONTENT.8.EE.B.5, CCSS.MATH.CONTENT.HSS.ID.C.7, and CCSS.MATH.CONTENT.HSA.CED.A.2.
Proportional Relationship Quiz - Measures the students ability to work with proportions and use units as a way to understand problems. This meets the standards CCSS.MATH.CONTENT.7.RP.A.3 and CCSS.MATH.CONTENT.HSN.Q.A.1.
Calorie Count Check - Students are required to record the calories they eat each day for the first two weeks of the project. The teacher will check the students progress tracking their calories each day, or when needed depending on the ability of the students to complete this task independantly.
Create a Meal Rubric - Students will use their knowledge of calories burned during exercise to create meals that fit given criteria.
Act 2 Guide Sheet - This guide provides the structure for Act 2. This includes a planning sheet for each group to divide up responsibility and plan tasks for each group member. Instructor can use this to follow group progress.
Survey Peer Evaluation Rubric - Students will create and administer a survey. Using the rubric the survey results will be peer evaluated. This addresses the standard CCSS.MATH.CONTENT.HSS.ID.A.1.

Blog Rubric - Students will add blog posts to the class Edmodo that will either reflect on what they have learned or answer a specific question (teacher choice). Students will also respond to peer posts.
Summative Assessments
Infographic
Students will create an infographic using an online tool of their choice showing what they have learned throughout this project.
Students will create a presentation that incorporates their infographic using their choice of online tools. These will be presented to all 9th grade Health classes, with local gym owners, restaurant owners and fast food restaurant managers invited to attend. Health teacher and math teacher will assess the students presentation according to the following rubric.
